CLINICAL COORDINATOR PHARMACIST:
The Pharmacy Clinical Coordinator provides pharmaceutical care to patients in accordance with federal, state, and local regulations and hospital policy.
This position works in collaboration with pharmacy leadership and pharmacy staff to ensure safe, cost effective, and appropriate use of medications.
The Clinical Coordinator accomplishes through planning, implementing, evaluating, and monitoring the pharmacy clinical programs, assisting in Drug Usage Evaluations (DUEs), collaborating with pharmacy management and medical staff, and serving as an educator, mentor, and resource for pharmacy staff, students, and residents.
Must be able to communicate clearly at all times and be able to prioritize projects to meet goals and established time frames.
This specific coordinator position is for a designated Pharmacy Clinical Coordinator for the Autologous Stem Cell Transplant and Cellular Therapy Program, and general Hematology/Oncology pharmacy practice as necessary.
The BMT Clinical Pharmacy Coordinator is an essential member of the multidisciplinary Transplant and Cellular Therapy program.
This coordinator will support the Transplant and Cellular Therapy program in both a hospital inpatient and outpatient clinic setting.
The BMT Clinical Pharmacy Coordinator is responsible for providing evidence-based, patient-centered oncology and supportive care pharmacotherapy management while serving as the subject matter expert for the multidisciplinary care team.
This includes clinical treatment assessment of patient regimens and supportive care guidelines; providing recommendations to physicians based on evidence-based medicine and/or national guidelines; monitoring patient medication regimens; serving as a medication information resource for patients, the pharmacy department and all members of the Transplant and Cellular Therapy care team; and assisting in transitions of care and patient/caregiver education, among other responsibilities.
