Registered Nurse (Pain Clinic)

The Pain Clinic Nurse provides consult management, education, case management and telephone triage duties, in addition to QI monitoring. He/she assumes accountability and responsibility for his/her nursing practice and decisions affecting patient care. The staff nurse considers all characteristics of the individual, including sex and sex role identification, age and life stage, state of health, race and culture, socio-economic status, values and previous experience with similar events, in order to assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential. Primary functions include patient assessment for specified clinic appointments, patient education and coordination of care within the clinic setting to facilitate the delivery of care in a timely and efficient manner. Patient safety is also a priority in the clinic setting with attention to implementation and maintenance of patient safety standards. Duties of this position may include but are not limited to:
Functions as the consult co-coordinator for the Pain Clinic and enters pain outcomes Questionnaire responses into the data base, performs follow-up with IR patients after injections, assists providers with procedures as directed. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Administers medications and procedures per established policies, protocols and guidelines. The staff nurse functioning as a preceptor assumes accountability and responsibility for the clinical development of assigned orientees. Consistently demonstrates sound clinical judgment and the ability to effectively provide and/or direct the care provided by team members for patients on an assigned clinic and modifies and updates the plans of care/treatment plans for patients on an assigned clinic. Identifies significant patient care problems and sets priorities for their management. Provides and directs nursing care for clients with complex nursing care needs; i.e. multi-system health care issues, complex psychosocial issues, etc. Demonstrates leadership qualities through collaboration with others. Participates in studying, identifying, evaluating and implementing creative process and system improvements that facilitate the Mission of the Medical Center at the program or service level and which can be documented through demonstrated outcomes. Demonstrates understanding of patient rights, policies relating to patient rights (i.e. Advanced Directives, DNR, patient confidentiality) and understanding of ethics for the Federal employee. Acts as a patient advocate, guides, directs or supports co-workers regarding ethical issues and makes appropriate referrals when issues are suspected or identified; i.e. knowledgeable regarding the chain-of command and ensures referrals are made to the Medical Ethics Committee when appropriate Demonstrates creativity in meeting patient needs. Identifies risk areas and analyzes resources available in the use of patient care equipment and body mechanics, while evaluating options available and selecting a course of action resulting in favorable patient/organizational outcomes. Verbalizes understanding of the process for advancement utilizing the Nursing Professional Standards Board process, the Nurse Qualification Standards and functional statements. Demonstrates knowledge of VA policies, procedures and clinical protocols and takes initiative in developing and evaluating unit level and/or disease specific standards of care, policies, procedures and nursing programs that guide nursing practice and/or measure performance. Demonstrates leadership by developing, revising, evaluating and communicating standards of nursing practice related to his/her area of nursing practice. Completes assignments in a timely and effective manner and follows safety protocols designed to reduce/eliminate employee risk of injury. Communicates effectively with co-workers and participate in staff meetings and unit in-services. Maintains effective communication with intra-department staff and demonstrate professional behavior by role-modeling, respect for others, collegiality, ability to accept constructive criticism and participate on ad hoc, nursing or medical center/health care system committees or task forces. Responsible for demonstrating and maintaining full competency in the use, pre-cleaning and/or reprocessing of reusable Medical equipment. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Licensure:
Must hold a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. Regarding this specific RN position, The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The VA Medical Center is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. BLS/ACLS REQUIREMENTS UPON SELECTION:
Per VHA Directive 1177, current certification in BLS issued from the American Heart Association or Military Training Network is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high-risk areas. If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification (issued from the American Heart Association or Military Training Network) prior to being boarded for an appointment with the VA. (Failure to provide such certification can result in delays in the hiring process).
