Licensed Professional Mental Health Counselor (Program Coordinator)

Specific Duties Providing services to, and oversight of, the GPD-funded community-based program(s). Verifying the Veteran status and eligibility of program participants. Verifying admission and discharge dates of program participants for billing purposes. Collecting and submitting GPD-funded program participant data as outlined by HOMES evaluation procedures. Complying with criminal conflict of interest laws and Executive Branch Standards of Conduct to avoid conflicts of interest in carrying out liaison duties. Assemble a team of subject-matter experts for initial and annual inspections. Forward the findings, of the complete inspection and/or re-inspection requirements to the required offices. Establish an accurate system of billing with fiscal service and compliant with the business practices of the medical center. Act as the primary resource for GPD Program information between the GPD-funded program and VA. Monitor the provider, at least quarterly, throughout the year for compliance to ensure the GPD-funded program is being administered as outlined in the grant proposal. Ensure all HOMES - VA Homeless Operations Management and Evaluation System, documentation is completed and forwarded to the appropriate recipient. Attend monthly liaison conference calls. Ensure proper program monitoring and mentoring. Forward, on a monthly basis, copies of per diem payment vouchers to the GPD Office. Providing oversight of GPD-funded program participants' care and, at times, case management of those participants at sites where organizations are receiving per diem payments. Case management is defined as the provision of services by VA clinical staff to homeless Veterans, which includes:
Monitoring the care in and assessing the compliance of the program receiving per diem as outlined in the recipient's original grant application. Intervening to facilitate compliance, or correction, of the program, when appropriate. Arranging, when necessary, and coordinating the care. Linking, when necessary, and referring to VA medical facilities, VA Regional Offices, and/or community agencies. Intervening, when necessary, and advocating on behalf of the Veteran to fill gaps in the delivery of services Clinical Relevance GPD Liaisons must ensure that their clinical documentation include the following on all Veterans in the clinical record:
An admission note that describes a plan of care which includes:
Reason for referral (including justification for any readmissions); Program to which the Veteran is admitted; Pertinent past treatment history; and preliminary treatment or service plan. Progress notes as clinically indicated and mandatory quarterly notes including progress notes. (that are related to the Veteran's progress toward the Veteran's goals), which include:
A discharge note; Summary of service; Description of aftercare plans. Work Schedule:

United States Citizenship:

Hold a master's or doctoral degree in:
Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling. CACREP defines the date when graduates are considered to have graduated from a CACREP accredited program. Additional information may be obtained from http:

Traditional Rehabilitation counseling programs that are accredited by CACREP do not meet the LPMHC qualification standards as Traditional Rehabilitation counseling differs from Clinical Rehabilitation counseling. Licensure:
Persons hired or reassigned to LPMHC positions in the GS-0101 series in VHA must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment. Preferred

Experience working with the homeless population. Experience working with substance abuse. Experience working with mental health. Experience working with community-based providers. Grade Determinations:
Social Worker (Program Coordinator), GS-12 Experience and Education:
In addition to the basic requirements, candidates must have at least one year of progressively complex experience equivalent to the GS-11 grade level. Experience must have been in a major specialty treatment program area such as, but not limited to, Post- Traumatic Stress Disorder (PTSD), mental health intensive case management (MHICM), or other areas of equivalent scope and complexity. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) In depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies procedures and or practice guidelines for the program. (b) Ability to make key decisions in the hiring and identification of top candidates. (c) Ability to provide leadership in facilitating the process of coordinating program consults, assessments, admissions, care assignments and discharges. (d) Ability to provide subject matter consultation to other colleagues and trainees within the field of counseling working on an interdisciplinary team. References:

The physical demands of this position involve active listening, reading, observing and evaluating verbal and nonverbal behavior, walking, speaking, writing reports, writing policies and procedures, writing clinical notes, using office equipment such as computers, telephones, fax machines, photocopy machines, and driving throughout the VISN.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
