Position Overview :
we are seeking a dedicated and compassionate Case Manager to join our team and work directly with homeless veterans to support their journey towards stable housing and employment.
The Case Manager will play a crucial role in assessing the needs of veterans, developing individualized service plans, and connecting them with resources and opportunities to achieve their goals.
Responsibilities:
Conduct comprehensive assessments to identify the needs and goals of homeless veterans.
Develop and implement individualized service plans in collaboration with veterans, focusing on housing stability and employment.
Provide ongoing case management and support to veterans, including advocacy, crisis intervention, and referrals to community resources.
Coordinate with internal and external stakeholders, including government agencies, employers, and community organizations, to facilitate access to housing, employment, healthcare, and other services.
Monitor veterans' progress towards goals, document case notes, and maintain accurate records in compliance with organizational and regulatory requirements.
Empower veterans to build essential life skills, such as budgeting, job readiness, and conflict resolution, through one-on-one coaching and group workshops.
Collaborate with interdisciplinary team members to ensure coordinated and holistic care for veterans.
Stay informed about relevant policies, best practices, and resources related to homelessness, veterans' issues, and social services.
Qualifications:
Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).
Minimum of 2 years of experience in case management, social services, or a related field, with a focus on serving homeless populations and/or veterans.
Knowledge of issues impacting homeless veterans, including trauma, mental health, substance abuse, and poverty.
Strong communication, interpersonal, and problem-solving skills.
Ability to work effectively with individuals from diverse backgrounds and build rapport with clients in crisis.
Demonstrated ability to collaborate with a multidisciplinary team and engage community partners.
Proficiency in Microsoft Office and case management software.
Valid driver's license and reliable transportation.
