Maintain a caseload of 25 families delivering on average 45 home visits a month Participate in required trainings and certifications through SMCS, Every Child Succeeds, and HMG Record participant progress toward goals in required databases and program documents within 48 hours of contact Connect families with community resources Be available one evening a week or a weekend day as needed to meet the needs of families that have daytime commitments Serve as an advocate to families Establish and maintain caring and consistent relationships with families through an open-minded and non-judgmental attitude Assist participants with resolving barriers that impact their goals OtherFunctions (include but are not limited to):
Contribute to effectiveness of the department and overall mission of Santa Maria Adhere to agency policy, procedures and the professional code of ethics.
Seek relevant training opportunities for professional growth and development.
Participate in regular staff meetings, staff training programs and supervisory sessions.
Satisfactorily complete any other special assignments as requested by supervisor Provide internal and external referrals for services Job Type:
