The Licensed Master's Social Worker assists patients and families by providing resources for care to enhance the patient's quality of life during adjustment, planning, and after end of life care.

Position
Qualifications:
1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care, or Acute Care environments.
LMSW required.
Bachelor's Degree Required.
License in Ohio Required.
Medicare guideline experience preferred.
Job
Responsibilities:
Assess and document patient/family social, emotional, financial, and psychological needs as well as resources on the Comprehensive Assessment.
Assist patient and/or family with establishing DNR, advance directives, and/or DPOA or Healthcare proxy as requested.
Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, and GIP as dictated by patient/family needs.
Work in both Hospice and Palliative care Programs.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
